One stir, one bang
On April 13, 1970, mission control asked the crew for a routine housekeeping task: stir the oxygen tanks. Damaged insulation on a fan wire inside tank two sparked, and the tank blew — taking the service module's oxygen, and with it the fuel cells that made the spacecraft's power and water. Jim Lovell's radio call became famous in slightly misquoted form; the actual words were 'Houston, we've had a problem.' The lifeboat improvisation
The rescue plan repurposed the lunar module — a two-person, two-day lander — into a three-person, four-day lifeboat. The crew splashed down safely on April 17, and the label 'successful failure' stuck because of what the mission proved: not the hardware, which failed, but the system around it — the simulations that had rehearsed disaster, the margins hidden in every design, the culture of working the problem instead of the panic. The accident investigation traced the tank's flaw to ground procedures years earlier, and the fixes reshaped how spacecraft are built and tested.
